Admissions Coordination and Assessment
The Admissions process is the first step in establishing services for new patients and their families. It involves giving an initial assessment and education about hospice services.
Home Care Teams
Highly skilled interdisciplinary teams offer a comprehensive and collaborative approach that focuses on pain relief, symptom control, comfort and support for patients and families. Services are provided throughout Maricopa County. More than 90% of patients are cared for in their own homes. Palliative
Care Units
Conveniently located throughout the Valley also provides services in acute care settings, skilled nursing facilities and residential/assisted living facilities-anywhere our patients call home.
Continuous Care Program
One-on-one care in a patient/family residence is available when skilled nursing care is required and the patient desires to remain at home.
Triage Team/After-Hour Support Services
The evening/weekend team is available around-the-clock to consult with families and assist them.
Administrative Services
Field staff members are supported by administrative teams in the areas of education and research, human resources, finance, information services, marketing, operations, intake and volunteer and support services.
